Piles or hemorrhoids are inflamed venous plexus in and around the anal region. It is predominantly classified into internal & external piles. The pathological symptoms depend upon the type of hemorrhoids present.

While internal ones are present with fresh bleeding during defecation via anus, usually without pain; external hemorrhoids are marked by swelling and significant amount of pain generally upon being thrombosed. Further, internal hemorrhoids are graded according to their various degrees of prolapse from Grade I-IV. An anal fissure (also known as fissure-in-ano or rectal fissure) is a condition characterized by a cut or tear in the anal canal region. In acute cases, it is marked by fresh blood from anal region while defecation. Few other predominant symptoms include pain after passing stool and persistent itching or irritation especially when chronic & infected.

Fistula-in-ano or anal fistula is an abnormal hollow pathway created between the epithelial layer of the anal canal & the perianal dermal layer. It is usually present externally as a small, swollen outlet on the skin surface. This opening when blocked (which is quite common) leads to formation of an abscess which drains inside the fistula tract. The predominant symptoms of anal fistula include a persistent pain in anal region, which is usually throbbing & worsens when in sitting posture, skin irritation around the anus, discharge of pus or blood from the affected area or even pain associated with bowel movements.
Abscesses can recur often in fistula, leading to accumulation of pus and further aggravation of the fistula. Sometimes even stool can pass through a fistula, causing infection associated with malodour & unhygienic conditions. If left untreated, they can cause systemic spread of infection.
