Sometimes, the condition is so bad that the man cannot even manage to have intercourse because he invariably ejaculates before he can get into the vagina. This means turning your mind to something else when you sense that a climax is near. For instance, you can: Think very hard about something totally unconcerned with sex. Some men try to treat themselves with a local anaesthetic gel. This product is advertised to the public as a good way to 'damp down' sexual sensation in the penis. But the local anaesthetic can 'dull' the sex sensation for the partner and secondly it can sometimes cause a distressing skin sensitivity reaction. The most important test is a simple & inexpensive “Semen Analysis” and for a semen analysis a fresh semen sample not more than half an hour old is needed after sexual abstinence for at least 2 to 4 days. If this test is done badly by poorly trained technicians in ill-equipped laboratories, the results can be very misleading. If the sample has less than 20 million sperm per ml, this is viewed to be a low sperm count and less than 10 million is considered very low and this deficiency is called ‘oligospermia’. Some men have no sperm at all and this deficiency is identified as ‘azoospermia’. Semen in these patients looks absolutely normal; it is only on microscopic examination that the problem is detected. Then it is found out whether the sperm is moving well or not- i.e. sperm motility. Normal shape of the sperm is also taken into account. During ejaculation the semen spurts out as a liquid, which gels promptly. This gel liquefies again in some 30 minutes so as to allow free motility to the sperm. If the gel is in thick consistency or fails to liquefy, it indicates a problem, normally one of infections of the seminal vesicles and the prostate. The presence of sugar called fructose is also considered. This sugar provides energy for sperm motility and its absence points to a block in the male reproductive tract. Researchers are of the opinion that toxic chemicals in our environment are one the causes for declining sperm counts and occupation hazards are also causing low sperm count. People become contaminated with the toxic chemicals via food chain due to the increasing use of synthetic pesticides & chemical fertilizers. Those who are working in places where there is much exposure to heat or chemical oriented industries are much prone to declining sperm count. However there are more than environmental and occupational hazards that threaten male fertility. There is a difference between nocturnal orgasms and nocturnal emissions because males can have a nocturnal orgasm (a sexual climax) without ejaculating (an emission of fluids, including semen). Females have nocturnal orgasms as well but there is no emission (as explained in another section). The important thing to understand is that there is nothing wrong with a male if he experiences a nocturnal orgasm or nocturnal emission, as they are normal and healthy experiences. The only problem with a nocturnal emission we can think of is the wet spot it leaves. Starting between the ages of 10 and 12, males begin going through the process of puberty. During this stage, teenage boys start to produce sperm and gain the ability to ejaculate. It is at this time that most males experience nocturnal emissions sometimes even before they learn about masturbation. While sleeping the male's genitals may rub against the bed or sheets or he may unknowingly stimulate them himself. When this happens, the male may become so sexually aroused that he has an orgasm (even if he is not awake to enjoy it!) along with a nocturnal emission. A boy's first nocturnal emission tends to cause the most confusion and worry. Most parents don't tell their sons what orgasms and ejaculations are before hand. Therefore, the first nocturnal emission is something that the adolescent boy is unfamiliar with. He may think something is wrong with his body: "Do I have cancer or some kind of disease?". He may fear that he has urinated in his sleep. Most adolescents don't want to have to explain to their parents why there is a wet spot in their bed and this causes fear as well. The truth is that nocturnal orgasms and emissions are natural and the wet spot will dry up quickly, though unfortunately it leaves a yellowish stain. Another confusing aspect of a boy's first nocturnal orgasm is the feeling of pleasure and arousal that he feels if he is awakened by it. If the boy has never experienced an orgasm before (via masturbation or otherwise), these pleasurable feelings are strange, yet at the same time enjoyable. Nocturnal orgasms may indeed lead a male to experiment with masturbation in order to experience those sexual feelings again. Teenage boys typically begin masturbating earlier than girls and do it more frequently which helps them learn more about sexuality and sources of arousal. While most parents do not teach their children about orgasms, let alone "wet dreams," an adolescent's friends and peers may attempt to play the Sexpert role. Knee pain is commonly caused by doing too much too soon when you haven't exercised for a long period of time - especially high-impact aerobics; walking, running or jumping on hard surfaces or uneven ground, excessive running up and down the stairs. Knee osteoarthritis is a common cause of knee pain. Osteoarthritis is a form of arthritis involving degeneration of the cartilage. Exercise is vital to maintain strength and flexibility of muscles supporting the knee, which reduces the stress on the knee joint. The risk increases with age. People with knee osteoarthritis may also need to take pain medications and/or other complementary pain treatments. Runner’s knee is a common cause of knee pain in young people (not just in runners). The pain is usually a diffuse pain behind the kneecap. Symptoms often worsen after climbing stairs, jumping, running or after a period of sitting. Stay away from high-impact activity if you have this condition. Prevent knee pain by keeping the muscles that support your knees strong and flexible. Walk before you run - before you engage in a strenuous high impact activity such as jogging or running, try walking for a week. Warm up and stretch before working out. Give your body a chance to recover from exercise. If you do high impact activities take every other day off. Doing knee exercises to strengthen and stretch the muscles that support the knee are vital for knee pain and injury prevention. Proper footwear is also important especially if walking or running on hard surfaces. Keep your weight under control. Reducing one's weight reduces stress upon the knee. Most knee conditions respond to a combination of non-invasive treatments such as applying heat or cold, temporarily restraining from activities that aggravate pain and medications that target pain and inflammation. The Greek word diabetes literally means“a passer through” or "a siphon". The condition was well known in ancient times and recognized by its polyuria and thirst often associated with emaciation. In 1674, Thomas Wills noted the sweet taste of the urine in some cases, making possible the first major sub division of the category into mellitus and insipidus varieties. Diabetes mellitus is defined as "a disorder of nutrition” in which sugar accumulates in the blood and is excreted in the urine, the daily amount of which is greatly increased. In comparison to people with no weight problem, generalized obesity made people only three times more likely to get diabetes. What is diabetes? Complications: In type I diabetes, deficiency of insulin secretion caused by destruction of the islets of langerhans is the major abnormality. In type II diabetes there is both insulin deficiency and insulin resistance. The association between these factors is not well understood. The etiology of diabetes mellitus is a complex interaction of genetic and environmental factors. The strength of the genetic factor is very variable. Until recently, type I diabetes has been regarded as a disease of acute onset. Prospective studies have now shown that people may have islet cell antibodies for up to three years before the onset of diabetes. It has also been shown that children may have glucose intolerance before developing symptomatic diabetes. These findings suggest that the clinical onset of type I may be preceded by a latent period during which islet cell antibodies and impaired glucose tolerance provide evidence of islet cell destruction. Two-factors,virus infection and auto immunity appear to be of importance in causing destruction of islet cells. If susceptible individuals can be identified during the latent period, the possibility exists of arresting the destructive process.

Type I diabetes is associated with a disease of the microcirculation, which is responsible for the complications of retintitis, nephropathy and some forms of gangrene and neuropathy. Electron microscopy studies indicate that the micro vascular abnormalities occur after the onset of the disease. Type II Diabetes is associated with disease of the macro circulation manifested by coronary artery and peripheral vascular disease. The fact that vascular changes often precede the onset of diabetes suggests that the metabolic and vascular changes occur independently. As mentioned above, the relationship between hyperglycemia and the increased mortality from cardiovascular disease is not clearly understood. The mortality rate is not fully explained by the known risk factors. Up to the present time, the life expectancy of patients with diabetes diagnoses before the age of thirty has been 30 to 50percent less than that of the general population. Death has been due to by renal disease in about 40 percent and by cardiovascular disease in most of the rest. There is increasing evidence that good control in type I diabetes is associated with fewer renal and retinal complications and this has now been made easier to accomplish by the introduction of insulin infusion pumps and frequent monitoring of blood glucose. The presence of microalbuminaria predicts renal disease many years later in both type I and type II diabetes. Photocoagulation in some types of early proliferative retinopathy can delay visual loss. The need for regular ophthalmologic screening of diabetics is now well established. Screening for microalbuminuria is not yet established as a routine procedure. The high degree of concordance in identical twins suggests that genetic factors play an important role in type II diabetes and that clinical diabetes is unmasked by environmental factors mainly in genetically predisposed individuals. This unmasking of diabetes by environmental change probably explains the emergence of diabetes as a common health problem in many groups of North American, Indians and the natives of some pacific islands. About 80% of patients with type II diabetes are obese at the time of diagnosis. The strongest predictor of type II diabetes is Impaired Glucose Tolerance (IGT) as defined earlier.
